Facilitate the vocational services to each individual receiving ACTT services who is interested in work. • Assist in the development of natural support networks. • Plan and develop activities that foster socialization and employment within the community to integrate individuals into society. • Provide monitoring and/or long-term follow-up for persons placed in employment and their employers. • Coordinate efforts with other providers of services as the North Carolina Division of Vocational Rehabilitation, LME staff, benefits counselors and family members to support the rehabilitation process. • Provide a variety of activities needed by the individual to develop the skills, behaviors and responsibilities necessary to live, work, learn, play and socialize in the community with the greatest possible degree of independence. • Teach specialized skill development to the individual in such areas as budgeting, cooking, shopping, dressing appropriately, bathing, using the telephone, writing letters, arranging appointments, developing leisure skills and recreational skills, and medication selfadministration. • Function as a member of a multi-disciplinary team to develop and implement treatment goals. • Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources.
