Participates in collaborative process facilitating recommended treatment plans to ensure appropriate, timely and cost-effective medical care leading individuals to maximum medical recovery and a successful re-entry into the work place. Coordinates healthcare services, including surgeries, therapies, pain management, and durable medical equipment, within a complex and fragmented delivery system. Ensures quality and appropriate level of care, prevention of delays, and resolution of issues improving patient outcomes. Maintains regular contact with involved parties and provides comprehensive timely reporting including current medical and vocational status, problem identification and case management recommendations. Communicates availability and coordinates early return to work strategies, such as light duty, job modifications, staged returns to work, etc, with treatment providers to promote a timely return to work, and provides accurate information regarding job requirements. Provides Catastrophic Case Management for individuals with complex diagnoses, often with critical health issues as well as multiple comorbidities. Performs home and work environment assessments after spinal cord injury to determine environmental barriers associated with transitioning to home and work and assists in removing physical environmental barriers through modifications or assistive technologies in order to increase function and independence in daily activities and occupations.Prepare videotaped recording and written job analyses allowing for accurate perceptions of the job requirements and physical demands of the job to be reviewed by the appropriate treatment providers to assist in coordinating treatment accordingly and making appropriate decisions related to prognosis for return to work.
