Provides support and assistance to the Assessment, Care Management, and program staff to ensure eligibility with HCBS programs- Monitor, document, and follow up with appropriate parties regarding eligibility for PASSPORT, Assisted Living Waiver, MyCare Waiver, Ohio Home Care Waiver, and Residential State Supplement programs- Utilize Ohio Benefits to track all waiver Medicaid applications until approved, denied, or closed and monitors Ohio Benefits and MITS to ensure continuous Medicaid eligibility once a member is enrolled onto a waiver program- Assist with processing and mailing out hearing rights for PASSPORT and Assisted Living Waiver programs- Monitor and document all missed services for all DHEO providers- Works to secure waiver service providers for members enrolled in PASSPORT, MyCare Waiver, Ohio Home Care Waiver, and Family Caregiver Support Program- Act as a liaison between DHEO teams and the County Department of Job and Family Services (CDJFS) and send Medicaid applications to appropriate CDJFS offices- Ensures compliance with all member and provider confidentiality according to HIPAA and Agency policies and procedures
