In 2015, I graduated from the HMS Fellowship in Patient Safety and Quality and have taken on progressive leadership roles to become the Associate Medical Director for Patient Safety. I have overseen greater than 125 root cause analyses that have resulted in 400+ organizational actions to help mitigate risk for patients. I have developed an enterprise-wide tracking system for action items that includes a dashboard, process for key stakeholder engagement and reports to hospital leadership. I have led multi-disciplinary workgroups to implement changes with direct impact on patient care. I currently lead the organizational Apparent Cause Analysis work and am co-chair of the ACA Review Committee. We leverage learnings from the ACAs to develop safety stories for organizational learning and develop operational actions to improve patient care.
