Provide education to patients and families by reviewing the hospice Medicare program, addressing misconceptions about the benefit, and dispelling any myths or questions regarding the sign-on process to ensure informed consent.Advance the admissions onboarding process by collecting information to determine eligibility, help develop a narrative that will inform an initial plan of care, and give the interdisciplinary team advance notice about immediate pain management, medical equipment needed, or complex family dynamics.Provide extensive long-term care education to facility staff and the community at large through presentations. Additionally, coordinate specialized training around hospice medications, body mechanics, and signs and symptoms of dying for assisted living communities, intermediate care facilities, and residential care communities as needed to ensure quality patient care and provide value to referring communities.
