Reimagining the care experience by leading the strategy & program development of Virtual Care at Hamilton Health Sciences. Our goal is to deliver an enterprise approach to virtual care best practice, ensuring an integrated & seamless delivery of quality care for patients, providers and the community. Leading the Virtual HomeGIM Clinic, an innovative program that supports capacity pressures/ bed flow across 3 HHS sites. HomeGIM reduces inpatient length of stay (LOS) by supporting medicine patients with virtual follow up appointments post-discharge. Most patients reduce LOS by 2 days (saving 180+ bed days to date!), freeing up beds for more acute patients across our sites.
