Most systems do not fail because requirements are unknown. They fail under pressure. In regulated environments, systems often appear compliant - until operational stress exposes what sits underneath. Not gaps in procedures, but gaps in how people respond, decide and act when it matters. These are not always visible in audits. I identify where systems are likely to fail under pressure - and help address those risks before they escalate. This sits at the intersection of system design human behaviour under pressurereal operational conditionsMy work typically includes: HUMAN FAILURE RISK DIAGNOSTIC A targeted review to identify hidden vulnerabilities in how systems operate under real conditions - not just how they are designed. FOCUSED INTERVENTIONAddressing specific behavioural and structural gaps that impact system integrity, decision-making and consistency under pressure. My background includes global technical leadership across certification bodies and manufacturing environments, with deep involvement in BRCGS, FSSC, GFSI-aligned standards and ISO frameworks. I have worked across audit delivery, technical decision-making, auditor calibration and system governance - seeing first-hand where systems hold and where they do not.
