At Valir PACE we bring an interdisciplinary team approach to caring for the complex geriatric patient. We provide a holistic medical model to support the LTC appropriate patient who wants to maintain independence in the community. It is here I have learned the nuance of navigating the complex care of geriatric patients with multiple comorbidities. We take on the 10% of patients who drive up 80% of hospitalization costs and utilize a value based health care delivery system to prevent such hospitalizations while on our services. Being PCP, I am honored to play a pivotal role in improving patient lives by managing medical care, educating patients on proper lifestyle choices, and reinforcing preventative strategies for managing chronic illness. Administratively, I have served on leadership committees and taken on the task of developing clinical approaches to enhance patient care, maximize reimbursements, and mitigate excess spending. If it weren’t for the love I have of the exam room and the relationship I enjoy with clinical staff I would pursue a corporate position. As it stands, I enjoy being the provider who makes corporate endeavors happen.
