Monthly comprehensive nutrition assessments and a nutritional plan of care for residents with significant weight loss, skin integrity issues, or receiving their food/nutrition parenterally or enterally(total parenteral nutrition or tube feeding)•Performing nutrition consultations requested by nursing staff and physicians and documenting medical records, educating the individual on how to make healthier food choices, how to manage any medical conditions through diet, and how to implement sustainable changes that fit their lifestyle. •Performing monthly sanitation audits to ensure kitchens and dining rooms are clean and in compliance with the National Food Code•Participates in performance improvement audits and on-going monitoring related to nutrition services, meal services, complaints or identified deficiencies. Assists in developing departmental policies and procedures.•Providing a pressure ulcer and weight report monthly to ensure plans of care are implemented and documented in the electronic medical records of each resident;•Conducting monthly in-service training to nursing and/or dietary staff on sanitation and nutrition issues;•Leading formal meetings with the Administrator, Director of Nursing, and Certified Dietary Manager monthly to discuss quality assurance indicators and results, training needs, and clinical recommendations and goals.
