Works hand in hand with clinical team to approve referrals for admission as well as the case management team for the continuation of authorization approvals. About the CLHF Setting: A Congregate living Health Facility provides a lower level of care then the Acute Care Hospital and higher level then a SNF. We specialize in Sub-Acute (Vent, Trach, and other high acuities needs that general SNF’s do not take) Our care is similar to a Sub-Acute inpatient setting but is in a residential home (setting). The home setting is a key factor in our high patient (and their families) satisfaction and quality of life.
