As a Permanency Specialist, my youth specific work includes monthly visits and case management, cross-team collaboration, child specific recruitment efforts to identify permanency for youth over age 10 and sibling groups, holding collateral meetings and coordination of transition information, full disclosure discussion, and post-placement support. Foster care licensing efforts includes intensive home study writing and interviewing, education on trauma and youth in foster care, understanding of statute and child welfare policy, matching process, transition planning, and post-placement and finalization support.
