ABOUT JORGE R. GARCIA

Jorge is a licensed Registered Nurse and currently serves as Director of Healthcare Services for Molina Healthcare of Florida where he oversees the transitions of care, case management and community connector operations for all of the plans product lines including Medicare Dual Special Needs Plan, Market Place, Medicaid and Long-Term Care. Molina of Florida currently provides managed healthcare services to members in twenty-six counties throughout the state of Florida including Comprehensive Medicaid & LTC members in Miami-Dade where the plan’s corporate offices are located. Prior to Molina, Jorge served as Director of Care Transitions for Independent Living Systems (ILS), a health management services organization in South Florida. In his role at ILS, Jorge was responsible for the Post-Acute Support Systems (PASS®) care transition program, and the organization’s participation in the national Community-Based Care Transition Program (CCTP). Jorge also had direct oversight of the implementation and training of the CCTP in 4 states, including Florida, New York, Alabama, and Chicago.
