About Jonathan C.

I am a board-certified cardiology clinical pharmacist utilizing a systematic approach to identify and implement data-driven solutions, advance patient care, and improve business outcomes.I completed a PGY1 pharmacy practice residency and PG2 cardiology specialty residency at the University of North Carolina Medical Center (UNCMC). I have worked as a cardiology clinical specialist since 2013 where I collaborate with interdisciplinary teams to provide exceptional patient care in the cardiac intensive care unit and acute care cardiology service. In order to expand my scope of practice and provide more complete care, I added cardiology ambulatory care responsibilities in March 2020.I also served as the Lead Pharmacist for the Heart & Vascular team and the Lead Pharmacist for all Acute Care Services for six years at UNCMC. In these roles, I personally oversaw and implemented 200+ quality improvement projects including heparin nomograms, targeted temperature management for post-cardiac arrest care guidelines, fibrinolytic guidelines, and a comprehensive direct oral anticoagulant (DOAC) guideline. Since medication errors related to anticoagulants were common, I developed and led a health system-wide initiative to implement anticoagulation prescribing and documentation processes within electronic health record to reduce medication errors, improve clinical decision-making, and close achieve regulatory requirements. Similarly, I designed and executed an A3 lean-six sigma project to improve use of intravenous immunoglobulin (IVIG) in the acute and ambulatory care settings. This project significantly streamlined the ordering of IVIG, optimized use of premedications, clarified infusion rates, and simplified dosing, and reduced medication costs.I have precepted and/or mentored 100+ student pharmacists, residents and/or new-hires. Through my extensive precepting, I have been honored to receive the Claude Paolini Preceptor of the Year Award (2020) and University of North Carolina (UNC) Region Preceptor of the Year Award (2016). Additionally, I also deliver 10+ lecture hours annually to 150+ first- and third-year student pharmacists at the UNC Eshelman School of Pharmacy.Throughout my career, I have been driven to improve patient outcomes utilizing an evidence-based approach. I believe there is no such thing as over-communication and that celebrating small successes begets larger successes. I believe in empowering others and creating collaborative atmospheres. I believe we must continue challenging the status quo in order to make true progress.
