   Promoted to lead position 12/2022.Provide services to high school transition clients, who have intellectual disabilities, which includes eligibility assessment and intake for Oregon Supplemental Income Program (OSIP), Presumptive Medicaid, SNAP, and the Oregon Health PlanAssess client needs and risk status through home visits, phone calls, and connecting them with resourcesDevelop comprehensive, client-centered care plans and monitoring funded services. Work extensively with supported employment providers and Vocational Rehabilitation to ensure individuals\' career goals come to fruition.Within the first year, I inherited a case load that was out of compliance with annual paperwork and client contacts.By 3 months working with my caseload, I was able to get everyone\'s paperwork completed and all required contacts made. Despite having multiple responsibilities, I have always been able to contact clients and providers within 2 business days to continue progress with desired outcomes and help others feel heard.
