About Joe Schum
I am a residency-trained internal medicine clinical pharmacy specialist with experience in a variety of disease states. I take pride in ensuring all of my patients are on the most appropriate medications that are individualized for their own unique disease(s). Proper transitions-of-care is at the core of preventing hospital readmission, ultimately saving both the patient and our healthcare systems financially. I prioritize patient transitions-of-care in the ambulatory setting by communicating with the patient, the provider, the pharmacy and the insurance company to identify which medications are covered and then to make sure the patient can get the medication and administer it safely. I have a passion to incorporate technology to enhance patient outcomes.I graduated from the University of Cincinnati with a PharmD and with a BS Chemistry. I completed my PGY1 pharmacy residency at Good Samaritan Hospital and completed a PGY2 Internal Medicine pharmacy residency at Good Samaritan Hospital.
