Responsible to identify strategic opportunities for IT enablement and develop business requirements in support of Regional Strategic Goals. The physician centric model to deliver health care services is now being required to make the shift to a consumer centric model aligning its care delivery around the patient and not the physician in order to deliver affordable care. In Business Development role, introduced the concept of breaking the continuum of care into four component continuums, as identified by Clayton Christiansen in Innovator’s Prescription: precision/ambulatory medicine, chronic condition management, wellness and disease prevention and Intuitive medicine. There are significant cost reductions of 20-60% in care delivery associated with breaking out these activities through business and financial modeling. Responsible to develop each business and financial model to identify the opportunity of an integrated care delivery and member centric approach. The models bring together and help identify how to manage multiple lines of care across the member centric activities eliminating any operational silo effects. The models define the appropriate channels, processes, tools, data and resources required by the activities.
