Liberty Community Services\' works with our partners to end homelessness in Greater New Haven - aiming to make it rare, brief, and one-time. We offer services to those experiencing homelessness and dealing with serious health and behavioral health issues. Our continuum of housing and support services reaches over people annually. Programs include supportive housing, prevention/diversion, and outreach and engagement services. We work to offer sustainable housing opportunities and engagement services for people experiencing homelessness so that we can be a stronger community, where everyone has housing and the necessary support to sustain it. Liberty is committed to implementing proven practices in our work—Housing is Healthcare.
