I oversee multiple teams executing population health initiatives for pediatrics including Intermountain Medical Group Primary Care and Maternal Fetal Medicine. I lead meetings with physicians, practice managers, and clinical staff to implement population health strategies. I collaborate with population health program vendors to increase enrollments, efficacy, and patient engagement in applications designed to reduce costs and healthcare system utilization. I contribute to the design of care delivery systems motivating patients to prioritize their preventative healthcare thereby improving health as well as reducing costs from an individual, organizational, and societal perspective. I have developed a unique analytical perspective that enables me to determine where energy can be focused on creating the most impact in patient-interactive web-based applications including but not limited to E-Asthma Tracker (designed for reporting symptoms on a regular basis keeping kids out of the ED) and CareCentra (an application designed for reporting symptoms on a regular basis preventing pre-term birth in high risk mothers). I oversee multiple care coordination teams who work with care managers and physicians to optimize preventative healthcare in pediatric patient populations. My team is expert in understanding care coordination needs from a social work standpoint. They have the skills needed to support and empathize with parents of special needs children and provide mental health and community resources to families in need. They multi-task a variety of support roles including scheduling and hosting their own virtual meetings with the healthcare professionals employed at the clinics they serve. I develop their workflow as well as write their training and continuing education material. I maintain the success of those campaigns by recruiting, training, and motivating my team to always strive for continuous personal and collective achievement
