ABOUT JESSICA HOHMAN
Jessica Hohman, MD MSc MSc is an experienced physician leader in driving value-based care & innovation across the care continuum. She is currently President of the Cleveland Clinic Accountable Care Organization. In this capacity, Dr. Hohman leads a broad population health portfolio—including network performance management and optimization, expansion of the home-based continuum, specialty-primary care VBC partnerships, transitional care and disease management programs, collaborative approaches to end-of-life care, and external partnerships to support care model innovation. Dr. Hohman has driven a performance turnaround—from a $10M loss in PY2018 to generating over $100M in total CMS savings and $55.7M in shared savings over the last several years. Dr. Hohman is also a grant-funded physician investigator in the Center for Value-Based Care Research & leads the Cleveland Clinic-Mayo Clinic Hospital Care at Home Consortium. She has presented at national meetings & co-authored publications on care model innovation, digital health, & post-acute care. She maintains an active Internal Medicine practice, & she is involved in the teaching & mentorship of trainees. Previously, Dr. Hohman was the Founder & Chief Medical Officer of CarePort Health, an end-to-end platform managing patient transitions across the care continuum & early pioneer in post-acute outcomes management before CarePort’s successful acquisition. Her prior work experience spans health technology, academic, & governmental sectors, including positions at LSE Health, the Massachusetts Health Connector, & the Institute for Strategy & Competitiveness. Her co-authored studies of value-based initiatives have been published in the Harvard Business Review & are used as teaching materials in Harvard Business School’s healthcare curriculum. A native of Akron, Ohio, Dr. Hohman attended Miami University on a full academic scholarship. There she graduated Summa Cum Laude with degrees in History & Chemistry. She subsequently earned her MSc Health Policy, Planning, & Financing & MSc Social Research Methods (Health Economics) as a Marshall Scholar at the London School of Economics. Dr. Hohman graduated from Harvard Medical School & completed Internal Medicine residency at the University of California-San Francisco followed by General Internal Medicine Fellowship at Harvard. She currently serves on the.406 Ventures Healthcare Executive Council, Medicare Advantage Leadership Council, & Harvard Medical School’s Alumni Council.
