Coordinate housing stabilization services for individuals and families within mixed-program sites (HCV, PBV, Tax Credit, RSP, and PSH).Conduct case management meetings to assess residents’ needs, create short and long-term housing goals, and connect them to community resources.Utilize Yardi, CRM, and Assess Force systems to document interactions, track rent ledgers, monitor inspections, and ensure program compliance.Collaborate with property management teams to address rent delinquencies, maintenance issues, and lease concerns before they escalate.Provide referrals to partner agencies (DHHS, RSP, MEAP, faith-based organizations, and nonprofits) for rental, utility, food, and mental-health assistance.Support residents through re-certifications, interims, and RAD transition paperwork, ensuring timely submission and accuracy.Serve as a liaison between residents, housing specialists, property management, and social-service providers to promote long-term housing stability.Monitor at-risk households and intervene early to prevent eviction or subsidy loss through targeted case plans.Maintain confidentiality and adhere to federal, state, and county housing regulations, Fair Housing laws, and HOC policy standards.
