I\'m a pharmacist (PharmD, CDP) who spent years in long-term care and geriatric pharmacy before transitioning to home care. I run Options in Greensboro and Burlington, NC.I go over medications with our clients and their families so everyone actually understands what\'s in the cabinet and why. I translate complex pharmacology into language families can understand and act on.I developed the MEDIC Framework — a systematic approach to evaluating medication-induced cognitive decline before assuming the diagnosis is dementia. I use it with every complex case that comes through our door.I also work with discharge planners, social workers, and physicians when they\'re dealing with complex medication regimens — especially when someone\'s on 10+ medications and something seems off.My focus: medication-induced cognitive decline, prescribing cascades, and the overlap between medications and dementia symptoms.
