Manage nutrition care for 150 residents at a 320-bed skilled nursing facility treating a multitude of medical conditions. Develop nutrition interventions promoting wound prevention and healing, improved labs, and overall nutritional stability. Lead the nutrition component of care for residents, including discharge planning and referrals to appropriate outpatient resources as needed. Partner with interdisciplinary teams of physicians, nurses, and rehab to continually assess resident nutrition status, advance diet consistency, and wean from tube feedings as appropriate. Conduct daily meal rounds to assess changes and update food preferences to encourage intake. Implement nutrition interventions as indicated based on clinical status. Update food service database to reflect changes in food and beverage preferences, nourishments/supplements, fluid restrictions, and tube feeding orders. Present nutrition education to sub-acute residents prior to discharge. Develop and participate in Nutrition Services events for staff and residents. Complete new admission, quarterly, annual, and significant change assessments. Provide training and mentoring to Stony Brook dietetic interns.
