Duties include facilitating the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. Evaluating the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations. Assist with the facilitation and implementation of timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care. Provide crisis intervention and support to patient/family support systems. Facility psychoeducational group therapy, while coordinating activities that promote quality outcomes for the patient throughout, while supporting a balance of optimal care and appropriate resource utilization. Identify any potential barriers to patient throughput and quality outcomes.
