Monitor various program systems needed to comply with Community Care Licensing Requirements (CCL) such as Urban Futures, and ACS Client rosters.*Assist Program Director in monitoring enrollments, and discharges from the program.*At the direction of ACS Supervisor, coordinate admission/discharge plan for clients.*Review all client submissions submitted to ACS program.*At the direction of ACS Program Director, interview potential clients to be admitted to ACS program.*At the direction of ACS Program Director, acquire additional potential client information from social workers, and collateral, such as family members.*Meet with all new ACS admits. Encourage their success in the program and reinforce program rules and expectations.*When ACS clients have case managers from other programs such as, Whole Person Care, FSP, or AB 109, continue to liaison with case managers to provide coordinated optimal care for clients.*Audit new ACS client files completed by line staff for accuracy.*Ensures that all ACS client treatment plans are updated every 60 days.*Maintain communication with Program Directors from Hillview Mental Health Center\'s AB109, FSP, FSP TAY and who monitor their clients for possible eligibility for the ACS program.*Assist Program Director in monitoring client status on an ongoing basis to determine which clients continue to meet ACS program eligibility and which clients should be considered for discharge.*Attends ACS and “Impact” meetings in turn with Program Director and CEO. Makes in-person reports to CEO to relay information from the meeting.*Assists Program Director in training new staff members in policies and procedures, documentation and providing direct services to clients.*Provides direct clinical services as much as possible to Hillview clients in any program.

Jeff Schubert is an all-around talent spending decades honing his craft and adapting his skills to the project(s) he is working on. Whether it is hosting the successful web show about the entertainment industry, Filmnut, that aired for 142 live episodes on TheStream.Tv, or the more recent Love Trumps All, a three-chapter satirical short film in which Schubert wrote, starred, directed, and produced, Schubert\'s passion and talent shine through. Jeff is comfortable leading or following, playing with others, or working alone. When he first moved to Los Angeles to pursue his career in entertainment it was important to him to earn his keep in an area he felt equally passionate about. And that is helping people. With that said, Schubert has over a decade of experience working with dual diagnosed mentally ill adults in an inpatient setting. Rather than take away from the arts, Schubert believes this experience adds to what he brings both in front of and behind the camera. He can see the humor, the crazy, and the humanity in ways perhaps other artists may not. And for this, he is grateful.
