Develops collaborations and builds trust throughout Collier County. Leads team to plan, staff and follow up on COVID-19 community testing, vaccination and health fair/screening events. Develops testing and vaccination schedule and communicates internally and externally with partners and the community. Manages the daily operations of the outreach team, to include hiring, training, daily supervision and coaching for neighborhood canvassing, testing events, vaccine events and other outreach. Works with the VP of External Affairs and Grants Director/Finance to manage grants, as appropriate. Ensures precision in tracking activities in the interest of federal grant reporting accuracy. Provides input in the development of community health equity initiatives including COVID-19 and other strategic health related-initiatives. Develops and maintains strong working relationships and regularly interfaces with clinical and operations team members to facilitate communication, clinical access and appropriate social support services for patients and prospective patients identified through outreach efforts. Supplies constructive feedback about community needs to appropriate HCN staff, managers, senior team members or community partners to ensure desired outcomes are met. Identifies and helps develop and implement organizational programs aimed at addressing the health disparity needs of our community. In collaboration with the VP of External Affairs develops relationships with community leaders and serves as HCN’s liaison in various community committees and organizations, especially related to social support services needed to address social determinants of health. Ensures community residents, guests and migrant farm workers have access to quality healthcare, regardless of ability to pay, or any residency or social determinant. Navigates patients to HCN for needed services, as well as any other systems that provide needed services.
