As a Pharmacy Intern, I call patients with chronic conditions, such as hypertension, diabetes, and asthma, in the ambulatory care clinic in order to improve medication adherence or potentially adjust current medication therapy. I also work with patients to find solutions to overcome socioeconomic obstacles that prevent them from achieving successful health outcomes. I counsel patients on new medications to ensure safe usage at home, including purpose of therapy, instructions for patients use, drug-drug interactions, and other special considerations. I also provide bedside counseling in the hospital and check-ins after discharge to address any patient concerns and support outpatient operations by filling prescriptions.
