My focus is on educating healthcare professionals and the community about the benefits and services of hospice care, ensuring those facing terminal illnesses receive the dignity and support they deserve.Key responsibilities include- Building and nurturing professional relationships with physicians, specialists, hospital case managers, skilled nursing facilities, assisted living facilities, and social workers to create a strong referral network- Identifying patients and families who would benefit from hospice services and guiding them toward appropriate care resources- Conducting educational presentations and outreach initiatives to raise awareness about hospice care, addressing both clinical and emotional needs- Collaborating with interdisciplinary teams to ensure seamless transitions and high-quality care for patients- Analyzing referral patterns and using data-driven insights to refine marketing strategies and enhance outreach effectiveness.
