Oversees 105 senior adult clients with severe mental health diagnosis, such as schizophrenia, bipolar, and major depressed mood disorder who have suicidal ideation and history of suicidal attempts• Supervises a team of clinicians, nurses, and substance abuse specialists who see clients in the community and provide case management, individual therapy, substance abuse counseling, and high-risk assessments • Provides daily consultation with clinicians on completing psychosocial assessments, client plans, CBT interventions, Motivational Interviewing, high risk interventions, as well as safety planning with clients • Ensures clients get connected to higher levels of care if needed, such as hospitalizations, Pert consultations, or locked facilities• Performs daily meetings with staff to review client contacts and provide solutions for high-risk clients who are struggling with daily functioning and mental health issues• Provides established outcomes driven psycho-educational counseling methods aimed at improving areas of need, treatment adherence, vocational/ educational rehabilitation and/or quality of life • Collaborates with Program Director/LCSW during weekly consultations for improvement of patient care and services as well as the Medical Team including the Psychiatrist and Registered Nurses•Completes disciplinary meetings with RNs, Psychiatrist, and Program Director to provide best practice with our client population• Collaborates with Pert and MCRT to ensure the safety of our clients as well as completes APS reports as needed• CASE Coach Program –The Chronological Assessment of Suicide Events. I currently attend the CASE training as a coach throughout the year. Case Approach is a flexible, practical, and easily learned interview strategy for eliciting suicidal ideation, planning, and intent.
