As Health Transformation Manager at the Willamette Health Council—the governance body for the Marion-Polk region’s Coordinated Care Organization (CCO), PacificSource—I lead multi-sector collaboration and strategic investment to advance community health outcomes for Oregon Health Plan members.I manage the Clinical Advisory Panel (CAP), a diverse group of leaders representing hospitals, FQHCs, public health, behavioral health, school health, dental, and managed care partners. Under my leadership, the CAP has overseen more than $4.3 million in Board-directed investments to improve care quality, address health equity, and build regional system capacity.In 2024, I spearheaded the launch of the Health Innovations & Investments Workgroup (HIIW), which introduced a “Shark Tank”-style funding model, enabling community organizations to pitch health improvement projects directly to local clinicians and health leaders. The result: a transparent, community-driven process that directed $2.4 million into high-impact innovations.In early 2025, I led the design and rollout of our region’s Social Care Impact Fund (SCIF), investing $1.9 million in projects that expand Medicaid providers\' capacity to conduct social needs screening, make closed-loop referrals, and deliver social-emotional readiness services for children. I facilitated webinars, developed grant evaluation frameworks, and coordinated training for our review teams.Beyond funding oversight, I serve as a strategic connector—bridging PacificSource’s managed care structure with the needs and insights of local providers. I facilitate CAP operations, drive alignment on policy and quality priorities, and host CAP events including our annual Clinician of the Year Awards and seasonal gatherings that strengthen cross-sector collaboration.At its core, my work is about designing systems that are accountable, responsive, and grounded in the lived realities of the communities we serve.

ABOUT JAMAL FURQAN
I’m a healthcare operations architect and execution strategist with more than 15 years of experience designing and implementing some of the most effective Medicaid, health IT, and alternative payment models in the country. My work has shaped Oregon’s healthcare landscape, influenced national policy, and proven that transformation doesn’t come from theory or slide decks—it comes from sitting knee-to-knee with frontline staff and building operating models that work in the real world.At the Oregon Health Authority, I helped design and launch the Patient-Centered Primary Care Home (PCPCH) Program, which saved the state $240 million in its first three years while strengthening prevention-focused primary care statewide. I then partnered with the Oregon Primary Care Association to lead the Advanced Payment and Care Model (APCM)—the nation’s first successful alternative payment model for FQHCs—generating $17 million in Medicaid savings while empowering clinics to hire behavioral health providers, nutritionists, and care coordinators to deliver truly team-based care.I co-designed the nation’s first 100% FMAP Tribal Care Coordination Reinvestment Program, restoring Medicaid funds directly to Tribal health systems, and helped position Oregon as one of the first states approved for the federal CCBHC demonstration, now a national model for behavioral health financing.Nationally, I’ve consulted with Medicaid agencies, health information exchanges, and public-sector innovators to secure tens of millions in federal funding, modernize payment and data infrastructure, and align clinical workflows with policy intent. Later, as Director of Client Relations at Bamboo Health, I led public-sector strategy for more than 25 state agency clients, helping bridge technology, policy, and operations.Most recently, I returned to Oregon to lead provider-level innovation inside the coordinated care model—working directly with clinics, hospitals, and public health leaders who all said the same thing: the system isn’t built for them to succeed.
