Participate on a multidisciplinary team to provide integrated behavioral health services to pediatric patients currently receiving primary care services within the patient-centered medical home model • Provide assessment, consultation and brief intervention for psychological, behavioral and medical disorders in order to improve psychosocial functioning and health • Provide behavioral intervention in chronic disease management • Collaborate with behavioral health program management to provide orientation, consultation and ongoing training to primary care staff to promote and ensure a team-based model of health care delivery
