ABOUT JADE UDAKA HUGHES
Wife, Mother, Functional Medicine RN. Currently I am an RN Care Coordinator with Simpra- Associated Care Ventures which is a Medicare Advantage Plan offered to patients in SNFs and in the community. I coordinate care and assess members to provide individualized care with a goal of reducing unnecessary hospitalizations and improving quality of life. I work alongside a local facility to provide education and support as needed. I am also a PRN RN Case Manager with ProHealth Home Health and Hospice and thoroughly enjoy meeting and caring for patients in their environment. My work history includes being the Director of Nursing Services- Certified (DNS-CT) in a 100+ bed long term skilled nursing facility in Gadsden, AL and acute care at DCH in Tuscaloosa. I am a graduate from the University of Alabama Capstone College of Nursing\'s BSN and MSN program of Clinical Nurse Leadership. The CNL is a master’s educated nurse developed by the American Association of Colleges of Nursing (AACN). The CNL is a leader in the healthcare delivery in all settings in which healthcare is delivered. The CNL assumes accountability for patient-care outcomes through the assimilation and application of evidence-based information to design, implement, and evaluate patient-care processes and models of care delivery (AACN, Oct. It is designed for the nurse who wants to make changes in nursing while remaining at the point of care, which is where I have enjoyed being most of my nursing career. Over the past 2-3 years, my love for root cause medicine has grown and become a passion. I recently obtained my Certified Functional Medicine Practitioner certificate with a goal of becoming a Functional Medicine Nurse Practitioner. I am passionate about geriatrics, home healthcare, psychiatry, research, and root cause medicine. What motivates me is hearing nurses say “we’ve always done it that way”. Healthcare is about finding new, better, more efficient, and cost effective ways to provide care.
