Jada Meads, MPH, CPH is a graduate of Washington University in St. Louis. Currently serving as a Community Health Worker/Women’s Intervention Specialist at The Community Wellness Project, Jada is pivotal in implementing programs funded by the Illinois Department of Health, Viiv Healthcare, Northwestern University, and The Center for Disease Control. Her primary focus revolves around enhancing the overall health outcomes of minority communities, with a specific emphasis on African American women.A Diligent self-starter, detail-oriented leader with an extreme passion for utilizing knowledge of program implementation to improve population health care in disadvantaged communities. Beyond her professional and academic pursuits, as a member of Delta Sigma Theta Sorority, Inc, she channels her energy towards community upliftment. Jada is also deeply invested in nurturing the next generation of public health professionals. She is dedicated to empowering, inspiring, and mentoring youth, advocating for non-traditional roles within the field of healthcare. Through her guidance and mentorship, Jada endeavors to instill a sense of purpose and drive in aspiring individuals, encouraging them to explore diverse career pathways that contribute to the advancement of black women in leadership positions.

   Manages and oversees a team of 5 Community Health Workers, ensuring they achieve quality improvement goals and deliver effective community health services.* Builds individual and community capacity through peer-led initiatives within the LGBTQ and minority communities* Certified in HIV / Hep C testing in Missouri and Illinois* implements HIV prevention programs including Counseling, Testing and Referral Services (both on-site and off-site), PrEP navigation services, interventions and education for priority high-risk population * Conducts education presentations in conjunction with HIV/HCV/STI testing with vulnerable populations.* Responsible for linking clients as they transition through the levels of care, ensuring that the treatment plan is formulated.* Ensures the facilitation and implementation of the treatment plan for the client’s service needs, including but not limited to scheduling appointments, assisting in completing applications, facilitating transportation, tracking appointments, and collecting information about the client’s progress.
