Responsible for leading care transformation and innovation through design, implementation and delivery of population health programs focused on quality of care, total cost of care, efficiency, and utilization across the network of providers and patients. · Oversees the development and deployment of a comprehensive performance improvement program focused on care delivery and total cost of care. Includes value stream program development, data analysis/reporting, data abstraction, and identification of Network opportunities for performance improvement. Incorporates elements of Lean, Six Sigma, and Project Management to lead a team that drives financial, clinical, and operational improvements across the Network. · Provides strategic oversight and leadership to a multi-disciplinary team of business analysts, performance improvement specialists, program managers, care managers/coordinators, and social workers aimed at delivering on organizational strategic goals. · Develops comprehensive strategy to position SWHR as a leader in quality and cost-effective care delivery across the CIN and the health plan. · Strategically plans care management (CM) models for new products and shifts in business priorities consistent with the Organization’s strategic plans and the needs of the Network. Conceptualizes, designs, and implements programs that include complex care, long term services, disease management, care transitions, and care coordination to meet the individual needs of the providers and patients. · Ensures that the programs are designed to meet all state and federal contractual requirements and the VP works in conjunction with the compliance office to oversee reporting, identify gaps and ensure that all policies, procedures, Models of Care, and other new requirements are met.
