In my role, I retrospectively review and assess inpatient medical records to ensure accurate DRG assignment, verifying that all principal and secondary diagnoses, including complications, co-morbidities, and procedures, are properly coded and sequenced in compliance with coding and regulatory guidelines. I formulate and submit physician queries when clinical judgment indicates that a diagnosis is not clearly documented in the medical record, requesting appropriate documentation to support the addition of any diagnoses. Additionally, I investigate and resolve inquiries related to record reviews, such as discharge disposition, admit/discharge dates, and invalid codes, from various departments to ensure timely and accurate reimbursement.
