   The Patient Services role is essential to provide Access Community Health Centers (Access) patients with comprehensive clinical care and ensure better health outcomes. This position collaborates with other clinical staff to assess patient needs and coordinate services. The main function of the Patient Services role is to provide case management to at-risk patients and/or prenatal patients and coordinate necessary follow-up care.This position is also responsible for tracking and reporting activities related to case management and referrals. The position also maximizes the financial benefits to Access and its patients. This position assists patients by pursuing their enrollment in government programs such as Medical Assistance and insurance on the Marketplace through the Affordable Care Act. It also assists under- insured patients by administering Access’s sliding discount programPopulation Health: Helps patients with resources for complex social issues, assists with referrals to home health and provides outreach efforts around clinical measures directed by clinical leadership to improve screening and vaccination efforts
