As part of Emoha’s mission to deliver “Elder Care at Home” with empathy and expertise, I lead end-to-end clinical and operational service delivery for home-based healthcare across diverse medical needs, ranging from general nursing to critical care.I oversee multidisciplinary care teams (Nursing Assistants, ANMs, GNM Nurses, Doctors, Therapists) to ensure timely, safe, and compassionate services tailored to each elder’s medical journey. I work closely with families, hospitals, and physicians to ensure seamless post-discharge care and long-term health management at home. Key Responsibilities:Managing critical care services at home, including ICU setup, BiPAP/Ventilator support, and chronic care.Acting as the bridge between hospitals, families, and Emoha’s clinical team to ensure smooth transitions and medical continuity.Leading a team of healthcare professionals and training them on clinical SOPs, emergency protocols, and elderly-sensitive care.Supporting families through case management, emotional counseling, and care plan coordination.Collaborating with doctors and hospital discharge units for referral management and service onboarding.Remote monitoring patient outcomes, Vitals levels, indwelling device care, and clinical escalations.Driving service quality, feedback loops, and helping continuously evolve Emoha’s elderly care model.Implementing digital care tracking and real-time reporting to enhance family confidence and clinical transparency.
