Every effort is made to keep the patient in his or her home environment. Delaware Hospice provides extensive bereavement care to our families for up to 15 months following the death of their loved one. In addition to ongoing hospice services, Delaware Hospice offers specialized programs including: the New Hope Program that addresses specific needs of children facing the terminal illness or death of a close relative. The Transitions Program enables individuals with a serious illness who are not appropriate for hospice services to access resources, information, and support services.
