The Manager, Community Collaborations and Corporate Seniors Strategy will be responsible for nurturing existing community partnerships and develop new ones that align with the strategic and patient population priorities of St. Joseph’s Healthcare Hamilton. At the system level this involves moving forward activities which are consistent with a transformational agenda to deliver better and more integrated care at St. Joseph\'s Healthcare Hamilton, and with community partners within the Greater Hamilton Health Network (GHHN). At the patient level this work focuses on planning, implementing, evaluating, and monitoring activities to ensure an integrated and coordinated approach to care is offered to patients who have unmet and/or complex needs within the current system. This position has specific operational accountabilities for the Nurse Led Outreach Team, Congregate Settings collaborations, and the SJHH Corporate Seniors Strategy.
