Here my work is heavily based in accordance with the Social Services and Well-being Act in Wales. I work primarily with older adults, though accept referrals from clients 18 years and over. Part of my role is to remedy problems related to clients home environment. This may involve the provision of equipment and/or the replacement of damaged aids. Alternatively, larger adaptations may be required to support clients to achieve their personal outcomes. The aim is to reduce risk and increase or maintain client independence for as long as possible. Where care is already provided by formal or informal carers within clients\' own homes I conduct moving and handling assessments which helps determine what alterations are required in the exsisting care package. Other aspects of my job involve supporting clients with housing related problems, the maintainence of tracking hoists where hoists are located in privately owned or rented properties, and wheelchair requests. Cooperative working is essential within this role. We work closely with housing surveyors, the medication management team, sensory support, care and repair, housing associations, ALAC, care agencies, gateway, social workers, assistive technology, sensory services, equipment stores and many more.
