Providing support to individuals with diverse mental health diagnoses, applying trauma-informed care to foster trust, rapport, and co-developed support plans to build resilience and healthy coping mechanisms- Process referrals and conducting risk assessments to ensure client\'s suitability for the service while maintaining client safety and adherence to service protocols- Delivering tailored 1:1 support, empowering clients to develop and apply coping strategies independently during times of crisis- De-escalated situations using least restrictive practices, demonstrating empathy, and applying active listening to provide immediate, person-centred support- Administered safeguarding protocols to protect clients from abuse and uphold their rights to wellbeing and safety- Collaborated with other team members, ensuring clear communication and continuity of support during daily handovers- Accurately documented 1:1 interactions and client progress in the company portal, ensuring records were comprehensive, accessible, and available for emergency use- Coordinate with the Kirklees Intensive Home-Based Treatment Team (KIHBTT), SWYP NHS Trust, and the Enhanced Care Home Team to deliver client-centered mental health care.
