My role is to support member practices in delivering high quality healthcare services in line with the Network Contract Directed Enhanced Service (DES). This involves conducting Structured Medication Reviews with our patients, actioning clinical letters, post-discharge medicine reconciliation and general medicine optimisation. I take a person-centred, holistic view of healthcare and look forward to developing in the role, maximising positive health outcomes for our patients.

ABOUT GARETH HARRIS
I am a dedicated and experienced Clinical Pharmacist with over 10 years in the profession, currently working with South East Shropshire PCN. My career spans both community pharmacy and primary care, where I have taken on a variety of roles in leadership, management, and clinical practice.As an Independent Prescriber and a graduate of the PCPEP pathway, I specialise in cardiovascular disease, running clinics focused on hypertension, CKD, AF, and cholesterol management. I am passionate about expanding my clinical expertise and am preparing to start diabetes clinics, with plans to enrol on a post graduate diploma in Diabetes in 2025 to deepen my knowledge in this vital area.In addition to my clinical work, I am committed to developing both myself and others. I have recently signed up as a visiting lecturer at Keele University’s School of Pharmacy and look forward to contributing to the development of future pharmacists. I have also mentored pre-registration pharmacists in the past and continue to mentor newly qualified pharmacists and pharmacy technicians, helping them to build their confidence and clinical skills.My passion lies in delivering patient-centred care while continually advancing my professional knowledge and supporting the growth of those around me.
