Full Time: Mental Health Social Worker in the WTC Environmental Health Center at Bellevue Hospital- Assesses new and current patients of the WTC-EHC, all of whom are survivors of 9/11, for symptoms of PTSD, anxiety, depression, and substance misuse using the PCL-C, HSCL-25, and CAGE Checklists- Assesses new and current patients for risk behavior and suicidal ideation, and intervenes as needed - Conducts psychosocial and benefits counseling sessions with new and current patients in order to assess the need for and provide appropriate social service referrals- Writes WTC Certifications for qualifying patients who express a desire for mental health treatment through the WTC-EHC program- Researches and provides community-based mental health referrals for patients as needed- Actively participates in bi-weekly disposition meetings with multidisciplinary WTC-EHC clinicians to discuss and make treatment recommendations for patients- Assists walk-in patients with community-based social service referrals as neededPart Time: Weekend Social Worker on adult inpatient medicine/surgery floors- Assesses recently admitted patients for psychosocial barriers and provides referrals/resources as needed- Coordinates and arranges discharge transportation for patients medically cleared to leave the hospital - Coordinates and arranges for patient community-based needs, ie. home health services, respite placement, shelter placement
