ABOUT FLORENCIA HALPERIN
I am the Chief Medical Officer of Form Health, where we provide evidenced-based obesity medical care delivered through telehealth. Our clinical care model consists of one-on-one, personalized longitudinal care from a dedicated ABOM-certified physician and a CSOWM-certified Registered Dietitian, who develop individualized treatment plans that address nutrition, physical activity, mindset shifts, and FDA-approved medications if appropriate. I joined Form Health in 2019 to develop and implement our clinical care model based on evidence-based guidelines and the best scientific evidence that we have to guide obesity care. It\'s been such a privilege to work with such an exceptional team of physicians, advanced practice professionals, and dietitians to deliver high-quality and compassionate medical care to people with obesity. We are thrilled to be expanding access to obesity care across the entire United States.I am an endocrinologist and a member of many professional societies including the American Medical Association, the Endocrine Society, and The Obesity Society. I\'ve been honored to be recognized at a national level for my dedication to patient care, including the Castle Connolly America’s Top Doctors, and the Schwartz Center Award for Compassionate Care.My team is hiring!
