Unified Care\'s solution uses telemedicine and remote vitals monitoring to connect the doctor\'s office and patient\'s home to provide patient-centered chronic disease management. iHealth Unified Care utilizes hardware, software and a physician-centered care team to provide a well-rounded care plan. The Chronic Care Management and Remote Patient Monitoring (CCM & RPM) program focuses on diabetes, hypertension, cardiovascular diseases, obesity and other chronic conditions. For the last few months, I have been working on the company\'s first CGM solution.
