Working with Advocate Aurora Community Relations and Community Health team members, the incumbent coordinates the development and implementation of report that tracks and measure outcomes supporting the Community Health Strategy. The reporting will reflect activities, investments and outcomes that support the improvement of life expectancy rates in targeted low income communities by five percent through six focus areas (housing, access to behavioral health services, access to primary medical home, community safety, workforce development, and food security).
