While most hospitals average 16% re-admission within 30 days of discharge, Segue Health average re-admission are less than 5%.We Stay In TouchWithin three days of discharge, one of our Segue Nurse Practitioners will make an in-home visit with the patient, to review Discharge Orders, provide Medication Reconciliation, and schedule follow-up appointment with their PCP. He or she will address any immediate problems or needs to make sure the patient completely understands all discharge orders.In-Home MonitoringA Segue Nurse Practitioner is assigned to the home for extensive monitoring that includes:* Comprehensive medication reconciliation* Safety check of premises* Confirmation diagnostic tests have been scheduled and performed* Review of test results* Answering questions and addressing concerns* Educating family or caregiverBody, Mind, and SoulWe care about the patient — body, mind, and soul.
