I support patients by ensuring safe, effective, and timely access to healthcare services. My responsibilities include: conducting patient assessments and identifying individual care needs, providing direct patient services like phlebotomy, performing ECGs, spirometry, wound dressing, suture removal, etc. Furthermore, I provide patient education, support, and guidance to promote self-management through face-to-face and telephone consultations. I frequently perform clinical reviews for patients with chronic conditions, particularly asthma, COPD, diabetes, and obesity, to monitor progress and compliance with care. I am also saddled with some administrative and audit roles, and maintaining accurate and confidential clinical records in line with NHS standards. These roles have strengthened my organisational, communication, and problem-solving skills while maintaining a patient-centred approach.
