We also offer services on a sliding scale for uninsured and underinsured. QUALITY. St Thomas CHC has been recognized by the National Council for Quality Assurance as a Level 3 Patient Centered-Medical Home. Level 3 is the highest level possible and is the Gold Standard in health care quality. SERVICES. St. Thomas CHC offers a full range of preventive and primary care services. In addition to the primary care services of Internal Medicine and Pediatrics, St. Thomas CHC also offers OB/GYN, Mammography, Optometry, Cardiology, Nephrology, Rheumatology, and Mental Health Services. APPOINTMENTS. The clinic offers same day appointments to established patients whenever possible, and strives to accept as many new patients each week as possible.
