About Eileen Koons
Eileen Koons is director of Huntington Health’s Senior Care Network, acommunity-based department, located in Pasadena, California. Senior Care Network provides home and community-based complex care coordination, care transitions/navigation, health education, information and resources to community-dwelling frail andfunctionally-impaired individuals and family caregivers.Ms. Koons serves in leadership and advocacy roles statewide and locally,including president of the MSSP Site Association board—the association of providers for California’s frail elderly Medicaid HCBS waiver program, Azusa Pacific University social work department advisory board member, and California Caregiver Equity Plan Advisory Group member.She was privileged to be selected as a Practice Change Fellow 2009-11, a national fellowship program for leaders in geriatric care funded by the John A. Hartford Foundation and The Atlantic Philanthropies. Ms. Koons previously worked for Los Angeles County’s Area Agency on Aging as planning supervisor and began her career in health and aging services as a medical social worker. She graduated with distinction from California State University, Long Beach\'s Master of Social Work degree program specializing in older adults and families and is certified by the Academy of Certified Social Workers through the National Association of Social Workers.
