Our \"Food is Medicine\" initiatives remove barriers to healthy eating to prevent and manage chronic disease and reduce social isolation. We do this by connecting locally-grown produce to health care services via fruit and vegetable prescriptions, cooking classes and food demos, and clinic-based gardens (more info at the link). I also our diabetes education activities that provides clinical services, group & individual education, and home visits to support patients with diabetes who receive care at any one of our three Yolo County health care centers.
